MSHA cites 3 companies for fatal trucking accident
CHARLESTON -- The federal Mine Safety and Health Administration has cited three companies for violations found while investigating the death of a novice trucker at a West Virginia coal operation.
Thirty-eight-year-old Danny Jones was thrown from the truck that hit a berm and was run over by its rear wheels Aug. 22. The accident occurred on a road from Moscow-based Mechel OAO's Double Bonus Coal's No. 65 mine in Wyoming County.
In a report Friday, MSHA blames poor maintenance, improper training and the truck's numerous safety defects, including missing seatbelts, missing door latches and a faulty transmission.
Officials with the companies -- Virginia-based trucking company Appalachian Leasing, subcontractor B&L Trucking and Double Bonus -- could not immediately be reached.
