W.Va. finishes revisiting 2005 coal truck accident

CHARLESTON — State mine inspectors have finished the investigation of a 2005 fatal coal truck accident.


They’re scheduled to present a report on the Nov. 8, 2005 death of truck driver Chad Cook to the state Board of Coal Mine Health and Safety Wednesday.


Cook’s death originally was not considered a mining accident. It was reclassified after state and federal mine safety officials decided the the accident occurred on the property of Mountain View Mine in Tucker County.


MSHA has since issued a report concluding Cook lost control of the truck, probably due to excessive speed.


State investigators also are scheduled to report on the death of an independent contractor crushed to death by a metal plate that fell from a bulldozer he was repairing last March.
