CRITICAL VIOLATIONS

Culloden Food Fair Deli, 2198 U.S. 60, Culloden, Nov. 5, 2, 3, Can opener is dirty. Floor is dirty in corner of kitchen. Chicken livers in display case are 108 degrees; sauerkraut and wieners is 108 degrees; fried chicken is 120 degrees; corn dogs are 130 degrees, jalapeno poppers are 120 degrees. Wall is in poor repair near toilet in men's room. Cheese, salad and dessert cooler is 50 degrees.

Harless Dining Hall, 1 John Marshall Drive, Huntington, Nov. 2, 2, 2, Ice cream freezer door in poor repair. Raw shelf eggs stored above beef product in walk-in cooler, items moved. Utensil trays are unclean; items washed. Melons were 49 degrees, items thrown out.

Rehab, 4th Ave., Huntington, Nov. 3, 2, 2, Floors in bar area(s) are unclean. Back alley has debris. Ice machine is dirty on inside. Wood blocks need removed in ice machine storage. 10 days.

Chili's Grill & Bar, 925 Mall Road, Barboursville, Nov. 7, 2, 2, Dumpster is open. Breading cooler is 56 degrees (unplugged) - everything moved to walk-in cooler. Door gasket on walk-in beer cooler is dirty. There is not chlorine in the sanitizing rinse -- corrected.

Shoops, 4th Ave., Huntington, Nov. 3, 5, 1, Items not date marked in cooler. Hand sink is blocked; items moved. Floor unclean in many areas of kitchen. Thawing chicken improperly in water; items moved and thawing corrected. Bathroom fixtures are unclean. Walk-in cooler flooring in poor repair.

Marshall Concessions, 1 John Marshall Drive, Huntington, Nov. 3, 2, 1, Light shielding missing in back room, in stand 2. Stand 5 sauce 120 degrees for hot holding; sauce thrown away. Pizza slices are uncovered in warming units.

Ginos Pizza of 14th Street, 1401 Washington Ave., Huntington, Nov. 7, 2, 1, Evidence of a pest problem found via mouse droppings under handsink in kitchen area. Front counter area is in poor repair and not easily cleanable. Door to the microwave is in poor repair. Will return to reinspect in 30 days.

Ray's Giovanni's, 1208 Trenol Road, Milton, Nov. 5, 1, 1, Vacuum breaker needed on outside hose faucet. Bake trays are dirty.

Johnson's Foodland Deli, 18 Perry Morris Square, Milton, Nov. 5, 1, 1, Door gaskets on walk-in freezer and walk-in cooler are dirty. Gnats are still present in restrooms.

Steak-N-Shake, 3409 U.S. 60, Huntington, Nov. 2, 0, 1, Nozzle for lemonade dispenser is dirty.

KFC Tri-State Foods, 315 Washington Ave., Huntington, Nov. 5, 0, 1, Coleslaw at 48 degrees, macaroni salad at 47 degrees and potato salad at 45 degrees at bar. Corrected on site, food discarded.

Kroger Deli, 5th Ave., Huntington, Nov. 8, 0, 1, Need the hand sink in the deli area installed back in place. Will follow-up with item in next inspection. Three months.

NONCRITICAL VIOLATIONS

Chick-Fil-A, 148 Melody Farms Road, Barboursville, Nov. 2, 2, 0, Cooler is prep area is 46 degrees -- 40 degrees, thermometer inaccurate. Shelf under hand sink in service area is dirty.

Cypress Manor, 7th Ave., Huntington, Nov. 7, 2, 0, Dumpster area is littered. Dishwasher leaks underneath.

Scatch's, 249 25th St., Huntington, Nov. 8, 2, 0, Utensils need to be stored properly. Need to remove all items from sink.

Max & Erma's, 16 Pullman Square, Huntington, Nov. 2, 1, 0, Freezer floor is in poor repair. Note: Previous critical violations have been corrected.
